What Is a UIL Sports Physical?
The University Interscholastic League requires every Texas student in UIL activities to be examined and cleared before their first practice, tryout, scrimmage, or performance. The signed form must be on file with the school before the student sets foot in an athletic period.
Three points parents miss most often:
- It is annual. A physical from a previous school year does not carry over, regardless of the date on it.
- It is not just for varsity sports. Marching band, cheer, dance, and drill team fall under the same requirement.
- The form must be on file first. Paperwork is due before tryouts, even when that sport’s season is months away.
The 2026 UIL Form Changed. Bring the Right One.
UIL revised the Pre-Participation Medical History and Physical Examination form for the 2026-2027 school year. Districts have been instructed to reject older versions, and an out-of-date form uploaded to Rank One will come back denied.
Two things to check before your appointment:
- Confirm the form says “2026” in the upper right corner of the Medical History page. Download it directly from the official UIL forms page rather than from a district site that may still host an older copy.
- Check the date requirement. Most Collin County districts will only accept a physical dated on or after May 1, 2026 for the 2026-2027 year.
Fill out the medical history side at home with your student before you arrive. It is the half of the form that most often stalls clearance, and completing it in advance keeps the visit short.

Many families combine the two. If your student is due for a well check anyway, ask when you book and Dr. Zaman can complete the sports clearance during the same appointment. For teens who need ongoing care beyond a single form, our adolescent care visits cover growth, mood, nutrition, and confidential concerns. Adults needing their own exam can book a yearly wellness visit separately.
What to Bring
- The 2026 UIL form, printed, with the medical history side completed and signed by a parent or guardian
- Your student’s immunization record (for annual check up). We do not do immunization
- Insurance card and photo ID
- A list of current medications, including inhalers, supplements, and anything taken for ADHD
- Glasses or contacts, if your student wears them
- Any brace, orthotic, or previous clearance letter from a specialist

Families travel to us from Murphy, Wylie, Sachse, Parker, Lucas, St. Paul, Allen, McKinney, East Plano, Richardson, North Garland & Plano. If you are coming from Wylie or Sachse, we are a straight run west on FM 544 and Brand Rd corner (behind the car wash).
If a cleared athlete later turns an ankle at practice, we handle sprains, strains, and minor sports injuries in the same office, so you are not starting over with a new provider mid-season.
